Forms

You may enroll in our courses on any given day regardless of semester or quarter because we have open enrollment.

We accept enrollments by mail, fax, or telephone. If you plan to enroll by fax or mail, please use the enrollment form available for download below. If you would like to enroll over the phone, please call (405) 325-1921 or (800) 942-5702 with your credit card number. If you are a University of Oklahoma student, however, we will need the signature of your college dean on an enrollment form before your application will be complete. This can be mailed, faxed, or brought into the office. We will also accept an e-mail confirmation that you have permission to enroll in the course.

The enrollment forms are in PDF format. You will need Adobe's Acrobat Reader to view and print the forms. Once you have the form downloaded onto your computer, you may complete the form onscreen by typing in the appropriate information and by clicking on the boxes or radio buttons.
