Lesson Cover Sheet

Our print study guides include cover sheets for lessons submitted. In addition to identifying the course and lesson submitted, the document should be completed with the students personal information (name, address, Sooner ID, and e-mail address/phone number). There's also a place for the student to include comments or questions. The instructor also takes a turn with the cover sheet, adding the lesson grade and comments to the student.
